Mental Health - Supported Community Living
The mental health Supported Community Living (SCL) program serves 140 people in Polk County that are diagnosed with chronic mental illness. The program is designed to support individuals to live as independently as possible in their own homes. Staff is available to provide individualized services at times that are convenient for the person and as frequently as the team deems necessary. The goal of the program is to maintain independent living and increase involvement in the community. Services are intended to provide education to help people develop or regain skills of daily living such as: medication support, health and wellness, socialization and leisure skills and crisis planning/support and problem solving. The program employs eight full time staff, including two Health Coordinators who focus their efforts on members with chronic medical issues including medication support.
